What done-right GLP-1 actually looks like

The medication is the easy part. The hard part is everything around it.

Muscle preservation. Without resistance training, ~25% of the weight you lose on a GLP-1 will be muscle. For a 200-lb man losing 40 lbs, that's 10 lbs of lean mass — which is exactly the wrong outcome. Three short strength sessions per week (45 minutes each) preserves nearly all of that muscle. Most clinics don't tell you this, much less help you build the program.

Protein intake. ~1g per pound of goal weight is the working target. GLP-1s suppress appetite enough that hitting protein targets requires deliberate planning — protein-first meal structure, supplementation when needed.

TRT pairing. For men with low or low-normal testosterone, GLP-1 + TRT produces dramatically better body composition than GLP-1 alone (more fat lost, less muscle lost, better metabolic markers). The two-prescription regimen requires a physician who can manage both.

Plateau strategy. Plateaus happen around month 4–6 and require a real diagnostic conversation: are you at dose ceiling, has compliance drifted, are you eating around the medication, do we need to switch molecules? Telehealth-only clinics rarely manage this well.

Discontinuation plan. What happens at month 12 or 18 when you decide you're done? Most patients regain ~⅔ of their weight within a year of stopping if nothing else has changed. The protocol should pair the medication with infrastructure that makes the loss durable past the medication.
