PRP Centrifuge Type Matters: Concentration Quality Differences

Two clinics can hand you something labeled PRP and deliver products that differ several-fold in platelet concentration. The centrifuge, the spin protocol, and the leukocyte profile decide what actually reaches the tissue. Here's how to tell a strong preparation from a thin one before you book.

A guy sat down in my Southlake office last month holding a printout from another clinic and asked a good question: "If PRP is just my own blood, why does it cost three times more at one place than another?"

Fair question. The answer is the centrifuge.

Platelet-rich plasma starts the same for everybody. We draw your blood, spin it, separate the platelet layer, and inject it. But the machine doing the spinning, the tube it spins in, and the protocol running on that machine decide how many platelets end up in the syringe. Two clinics can both hand you something labeled PRP and deliver products that differ several-fold in concentration. Same name on the receipt. Very different biology going into the tissue.

I still round as a hospitalist in DFW, so I think about dose constantly. Dose is the whole game with the P-Shot. Here's what separates a real preparation from a thin one.

Does the Centrifuge Actually Change What Ends Up in the Syringe?

Yes, substantially. Spin speed, duration, rotor design, and tube geometry all determine how cleanly platelets separate from red cells and plasma. Published comparisons of commercial PRP systems have found final platelet concentrations ranging from barely above baseline to more than seven times baseline, starting from the same volume of donor blood.

Centrifugation is density separation. Red cells are heaviest and pack to the bottom. Plasma is lightest and rises. Platelets sit in a thin band called the buffy coat along with the white cells. The operator's job is to capture that band without dragging red cells along or leaving half the platelets behind.

How well that works depends on details patients never hear about. Relative centrifugal force, measured in g, matters more than the rpm on the display, since the same rpm produces different force depending on rotor radius. Fixed-angle rotors pellet differently than swinging-bucket rotors. Some tubes use a gel barrier that holds the layers apart once spinning stops. Others rely on a technician drawing off the right layer by eye.

Change any one of those variables and you change the product. That's not a knock on the science. It's the reason you should know what your clinic is running.

What Counts as a Therapeutic Platelet Concentration?

Baseline whole blood runs roughly 150,000 to 400,000 platelets per microliter. Most regenerative protocols aim for about one million per microliter, which is four to six times baseline for the average man. Preparations that land near twice baseline are delivering far less growth factor signal than the research protocols used.

Platelets matter because of what's stored inside them. Their alpha granules hold PDGF, VEGF, TGF-beta, EGF, and IGF-1. When platelets activate at the injection site, they release that payload over the following days, which recruits repair cells and starts new blood vessel growth.

VEGF is the one I care about most here. Erections are a vascular event, so angiogenesis is the mechanism that moves the needle. More platelets means more VEGF, and that holds up to a point. Past roughly six to eight times baseline the added benefit flattens, which is why we work to a target rather than a "more is better" rule.

Why Volume Isn't the Same as Dose

This trips up men comparing quotes. A clinic advertising 10 mL of PRP sounds like it's giving you more than one offering 5 mL. But total platelet dose equals concentration times volume. Five milliliters at six times baseline delivers far more platelets than ten milliliters at one and a half times baseline.

When you compare clinics, ask about the multiple, not the milliliters.

Single-Spin or Double-Spin: Which Protocol Produces Better PRP?

A single soft spin separates red cells and typically yields two to three times baseline concentration. A double-spin protocol adds a second, harder spin that pellets the platelets so excess plasma can be removed, commonly reaching five times baseline or more. Double-spin takes longer and demands more careful technique.

The second spin is where the concentration gain comes from, and also where a preparation gets ruined. Spin too hard and you damage platelets or trigger early activation, which dumps growth factors into the tube instead of your tissue. The pellet has to be resuspended gently in a small volume of plasma, then injected reasonably soon after.

Anticoagulant choice matters too. ACD-A preserves platelet function better than plain heparin here, and it buffers pH, which changes how platelets behave once they hit tissue. Small detail, real consequence.

Leukocyte-Rich or Leukocyte-Poor: Which One Belongs in Penile Tissue?

Leukocyte-rich PRP keeps the white blood cells, which adds antimicrobial and catabolic signaling that suits tendon and some joint applications. Leukocyte-poor PRP removes most of those cells for a gentler inflammatory profile. For penile and other delicate soft tissue, leukocyte-poor preparations are what most protocols call for.

Neutrophils bring matrix metalloproteinases and interleukin-1 beta with them. In a chronically degenerated tendon, that controlled inflammation can restart a stalled healing process. In erectile tissue, where the goal is new microvasculature and healthy smooth muscle, the same signaling works against you and tends to produce more soreness for no added benefit.

How Do Closed Systems Compare to Bench-Top Tube Kits?

Closed systems process your blood inside a sealed disposable, so the sample never meets open air and the protocol is fixed by the device. Open tube kits depend on an operator pipetting the correct layer by hand. Closed systems produce more reproducible platelet yields and a cleaner sterility story, which is why most physician practices use them.

Reproducibility is the quiet advantage. With an open kit, your result on Tuesday depends on who's working Tuesday. A sealed system running a validated protocol gives roughly the same output every time, so when we track a man's response across sessions we're comparing like with like.

One point stated plainly, then set aside. The FDA clears PRP devices for preparing platelet concentrate from your own blood. It has not approved PRP as a treatment for erectile dysfunction. Using that concentrate in penile tissue is off-label, supported by a growing set of randomized trials and a decade of clinical experience. That's the honest evidence picture, and it's precisely why preparation quality deserves your attention: when benefit depends on delivered biologic dose, how that dose gets made is not a trivial detail.

What we see clinically is straightforward. Men who get a properly concentrated, leukocyte-poor preparation report firmer and more reliable erections over the following two to three months more often than men who got something thin. What Should You Ask Before You Book a P-Shot Anywhere?

Ask what platelet concentration the system produces relative to baseline, whether the preparation is leukocyte-poor, and who performs the injection. A practice that measures its own output can answer in numbers. Also ask whether a hormone and vascular workup is included or whether you're purchasing an injection by itself.

Here's the list I'd hand a friend:

  • What multiple of my baseline platelet count does your system produce, and how do you know that?
  • Is the preparation leukocyte-rich or leukocyte-poor, and why did you choose that for penile tissue?
  • Single spin or double spin, and what total platelet dose gets injected?
  • Who draws the blood, who processes it, and who performs the injection?
  • Does my visit include morning total and free testosterone, fasting glucose or A1c, a lipid panel, and a blood pressure check?

That last one separates a medical evaluation from a menu item. Who Gets the Most Out of a Properly Concentrated P-Shot?

The strongest responders have mild to moderate vascular erectile dysfunction, some preserved morning erections, and metabolic health that's either decent or actively being treated. Men in their forties and fifties who still get partial benefit from oral medication and want to depend on it less tend to see the clearest gains.

If that sounds like you, concentration matters enormously, because you've got healthy tissue and a real angiogenic response available. Give that tissue a strong dose and it answers. A few men need a different plan first, and this part isn't negotiable. Active genital infection, thrombocytopenia or a known platelet function disorder, anticoagulation that can't be safely paused, and active hematologic malignancy all take PRP off the table for now. We hold NSAIDs for about a week before the draw since they blunt platelet function, and we look hard at anyone on daily antiplatelet therapy. Does a more expensive PRP kit always mean better platelet concentration?

Not automatically. Price reflects the device and disposable, not the result. Ask for the concentration multiple and the leukocyte profile. A practice that measures its own output can tell you both without hesitating.

How many platelets should a P-Shot actually deliver?

Most protocols target around one million platelets per microliter, roughly four to six times your baseline. Total dose depends on both concentration and injected volume, so ask about each one.

Is leukocyte-poor PRP better for erectile tissue?
