Testosterone replacement therapy (TRT) is the medical reintroduction of testosterone in men whose endogenous production has fallen below physiologic thresholds, with documented symptoms. The standard threshold for clinical hypogonadism is two morning total testosterone readings under 300 ng/dL paired with documented symptoms—low energy, low libido, depressed mood, poor recovery, increased visceral fat. We treat people, not lab numbers, but we also do not prescribe TRT to men with normal labs and no symptoms.

The default delivery method at our clinic is weekly testosterone cypionate injections in-clinic, which produce stable serum levels with the simplest dose calibration. Topicals (cream or gel) are useful when injections aren't preferred. Pellets (every 3 to 6 months) offer convenience but the dose can't be adjusted once placed. We discuss all three at the first visit and choose based on schedule, response, and labs.

Monitoring is what separates done-right TRT from franchise-clinic TRT. Our protocol checks total and free testosterone, SHBG, sensitive estradiol, hematocrit, prolactin, PSA (men 40+), comprehensive metabolic panel, and lipid panel at baseline, six weeks, and quarterly thereafter. Most franchise clinics check total testosterone alone. The difference shows up over years.
